DHS reserves the right to cancel the registration of persons who register without advance authorization.
Registration rates for the 2011 CAO Forum are as follows

Payment
DHS Employees
Upon approval of the Purchase Card Holder for your area - you may register without providing credit card information.
Select the payment option of "Purchase Card Holder Name" and then enter the name of the Purchase Card Holder who will be responsible for payment of your registration fee.
The Purchase Card Holder will make payment for all registrations by January 5, 2011.
Invited Guests (who are not DHS employees)
Payment via credit card is required. Purchase Orders and Checks are not accepted. Your credit card will be charged upon completion of your registration.
Invited Speakers
Select the Speaker Registration option and payment is not required.
Cancellation Policy
Refunds for the registration fee, minus a 15% handling charge, may be given if your cancellation request is received by A-S-K Associates, Inc. no later than December 17, 2010 at 5:00 p.m. CST.
After December 17, refunds will not be available.
Attendee Substitution Policy
Attendee substitutions (name change from someone who cannot attend to someone who has not registered yet and can attend in his/her place) may be made at any time, provided a written release from the original registrant accompanies the substitution. Hotel Cancellation
All hotel reservations should be cancelled by the individual, directly with the hotel.
